 Athletics- Winter 2020 Consent to Participate- Communicable Disease Waiver

Required: Face coverings - 2 reusable, washable. Images on coverings should follow the district dress code. According to the Center for Disease Control: cloth face covering should: fit snugly but comfortably against the side of the face; completely cover the nose and mouth; secured with ties or ear loops; includes multiple layers of fabric; allows for breathing without restriction; can be laundered and machine dried without damage or change to shape. Face shields that cover the nose and mouth and can be cleaned are acceptable.

  If you are interested in participating in a sport at KHS, please print off the following forms, sign and submit to the main office.
Mandated Paperwork List - all must be turned in before an athlete can tryout.

To Register for sports complete your 2019-20 online registry: (all KHS students must have an online registry completed before school starts)
